Fee Disclosure:

• Full service Doctor's recommendation and OMMA submittal is $190.

• If you have Medicare / Medicaid / Soonercare the Full Service fee is $90.

• If you are a 100% Disabled Veteran the Full Service Fee is $75

• If you are a Veteran, but not 100% disabled you qualify for $10 off.

• Full service means that we submit the documents to OMMA on your behalf.

• If you are a minor, or need a medical card on behalf of a minor the fee can be up to $500.

• Out of state cards do not qualify for Medicare or Medicaid reduced rate state fee.

Breakdown of Fees:

• The Doctor's Fee is a $35 base price.

• The Oklahoma Medical Marijuana Authority Medical Card State Fee is $104.30.

• If you have Medicare / Medicaid / Soonercare the OMMA State Fee is $22.50.

• If you are a 100% Disabled Veteran the OMMA State Fee is $22.50.

• Out of state card fees are the same, but are only good for 30 days (1 month).

Required Documents:

• Photo of Passport, Oklahoma State ID or Driver's License (address can't be a P.O. Box).

• If it is a P.O. Box or a Passport for an ID document, we will need a document proving residency.

• Acceptable residency documents: utility (gas, electric, water or trash - not a phone bill), deed or a lease agreement with correct matching name.

• If you have Medicare / Medicaid / Soonercare, we will need a copy (photo, scan or screenshot) of your card or benefits letter.

• If you are a 100% Disabled Veteran, we will need a copy of your VA ID or a copy of your benefits letter stating you are 100% disabled.

• We will also need a passport style photo of you on a plain white background, with no glasses or hats being worn in the photo.
